School-Based Clinicians follow Eagle County School District’s calendar which includes holiday breaks throughout the year.

Proficiently evaluates incoming cases to provide a high level of service in an efficient, effective, and compassionate manner.
Collaborate with school administrators and relevant staff members to receive referrals for students who may benefit from behavioral health services.
Understands the need and sensitivity of immediate response to students.
Performs initial clinical assessments on all referred students to determine degree of risk and document appropriate intervention plan and level of services needed.
Collaborate as needed with first responders to manage and navigate ambiguous emergency crisis scene along with medics, officers, investigators, while supporting relevant members with continuous updates and intervention as needed.
Partner with family members or relevant emergency contact involved during management of a crisis event.
Skillfully provide immediate crisis intervention services, mental status exam and assessment of any other psychological and physical needs.
Obtains appropriate paperwork and maintains strict adhesion to confidentiality and record keeping.
Counsel, educate, and provide follow up information to students and designated contacts to assist them in understanding, dealing with, or supporting students participating in school-based therapy services.
Refer patients, clients, or family members to community resources or to specialists as necessary and maintain client contact and follow-up.
Utilize evidence-based clinical interventions, including development of treatment plan, for students receiving ongoing therapeutic services.
Ensure proper documentation, completion, system update, and communication regarding all cases.
Performs other duties as assigned that may be inherent in the position.
Learn about new developments in counseling by reading professional literature, attending courses and seminars as well as establishing and maintaining contact with other social service agencies.

Knowledge of DSM-IV 5 diagnoses and symptoms.
Working knowledge of clinical assessment.
Knowledge of local and agency resources for referrals.
Strong verbal, written, and interpersonal skills.
Ability to adapt to change in a fast-paced environment, under minimal supervision.
Ability to maintain regular communication with clinical team.
Ability to maintain boundaries with clients, practitioners, agencies, and community members.
Ability to always maintain composure, professionalism, and structure.
Ability to navigate ambiguous situations during an emergency.
Skilled in basic computer operations, data management, and software.

Master’s Degree in Counseling, Psychology, Social Work, or related field.
Colorado state license required.
Two years of experience in counseling related role and crisis or trauma assessment experience preferred.

Your Hope Center is a nonprofit mental health agency that has been serving the community since October of 2018 with two separate programs: mobile crisis co-response and school-based therapeutic services. Responsibilities Include:

Provides short-term intensive in-home and community-based stabilization services.
Provides brief youth, individual, and family therapeutic support and psychoeducation, skills training, and therapeutic clinical interventions that may include: safety planning, motivational interviewing, cognitive and behavioral therapy techniques, communication skills, boundary setting, etc.
Completes a strength-based assessment which includes administering assessment measures and works collaboratively with The Hope Center team and community partners to formulate a culturally relevant crisis plan and plan of care.
Initiates contacts with other agencies and service providers in Eagle County to develop and maintain resources for individuals with mental health challenges and their families.
Demonstrates provision of a strengths-based philosophy to help individuals and families meet their needs and realize their vision of the future.
Provides phone and on-site crisis assessment, intervention and stabilization services in accordance with the needs of the individual and family; includes ability and willingness to work some evenings and weekends.
Assists with the coordination of community-based alternatives (i.e. transportation, in- home supports, Intensive Outpatient Program, etc.) to inpatient hospital admissions.
Assists, when indicated, with placement of individuals in inpatient hospital facilities and works closely with inpatient facility staff toward timely discharge by coordinating community-based transition and stabilization plans.
Evaluates progress in reaching goals and assists with development and implementation of transition planning.
Serves as a discharge point of contact for anyone returning to our community from a higher level of care.

Completes all necessary referral/enrollment paperwork with the individual/family.
Completes all program participant-related paperwork (i.e. assessments, crisis plans, safety plans, plans of care, service authorizations, case notes etc.) according to established timelines.
Completes all program-related paperwork and enters information in the data system in a timely manner.

Works collaboratively as a member of the Hope Center Team, including providing case coverage when absent and sharing coverage for others’ absences.
Continually develops clinical skills through regular clinical/reflective supervision, group case consultation, training, and independent study.
Works to establish close relationships with referring agencies such as inpatient behavioral health facilities, law enforcement, the court system and/or local jail or state hospital systems to develop a process for receipt of these discharge referrals.

Engage in ongoing examination of and engagement with one’s own biases and continued work toward cultural humility.
Work with everyone embracing different perspectives and life experiences beyond one’s own experience and perspective.
Attends and actively participates in program meetings, required training, and agency meetings.
Follows confidentiality guidelines to ensure consumer privacy.
Follows policies and procedures as cited in the Employee Handbook.
Performs other duties as requested by the direct supervisor or agency director that enhance the overall agency operation.
Holds a valid driver’s license, has access to reliable transportation, and has a driving record that enables purchase of automobile insurance, in accordance with agency guidelines.

Qualifications Include:

Master's Degree in counseling, social work, psychology, or closely related field.
Current training license or ability to attain within 3 months of hire (LCSW, MFT, LPC).
Daily access to a reliable vehicle, with valid driver’s license and insurance.
Demonstrated written and verbal communication skills.
Demonstrated experience working with individuals or families on issues related to mental health, community resources usage, case management and advocacy.
Experience working with others from diverse cultural backgrounds.
Commitment to strength-based, recovery-focused, trauma-informed programming.
Demonstrated ability to work as a member of a team and maintain positive collaboration with others within and outside an organization.
Proficient computer skills, and willingness to learn new skills as needed or required.
Ability to pay attention to detail and a commitment to quality and the timely completion of duties.

Clinical Licensure (LCSW, LPC, LMFT)
Knowledge of Eagle County's community resources.
Demonstrated flexibility, initiative, and creativity.
Prior experience providing crisis stabilization services.
