School-Based Programs

In partnership with Eagle County Schools, Your Hope Center’s school-based clinicians address the rise of social emotional challenges facing children and teens through access to walk-in therapeutic services in thirteen schools in our community

Students can receive counseling, at no charge, throughout the school day from Your Hope Center’s school-based therapists. Our therapists work with teachers, administrators, and school counselors to provide direct support to students with individual or group counseling sessions.

Access to therapy during the school day removes scheduling and transportation barriers and reduces the stigma of receiving mental health support.

Community Stabilization Program

The Community Stabilization Program gives individuals in crisis an opportunity to remain in their homes while receiving treatment. By teaming with the natural supports in each person’s life, the Community Stabilization Program provides hope, connects people with services, and helps individuals heal so they can look toward a brighter future.

These services are intended to prevent psychiatric hospitalization and keep people at home in their natural environment while they stabilize and heal through a set of diligently followed protocols to ensure their safety.

Desired Outcomes

  • Reduced use of inpatient psychiatric hospitals by keeping individuals at home

  • Individuals have more control over their treatment plan

  • Fewer days out of work and fewer days removed from the community

  • Lower out of pocket cost for stabilization

  • Rapid resolutions of crises

  • Swift linkage to treatment services

  • Establish new relationships for camaraderie and ongoing support

  • Increase community confidence in handling crisis situations

  • Demonstrate the power of community support to people in crisis

  • Reduce the recidivism rate of individuals seeking crisis assistance

The Community Stabilization Team is available weekdays, 8:00 am - 5:00 pm. Call us at (970) 306-0674 to get started.
